What to Do if You’re Injured on Set

In the unfortunate event you are injured on a set location, AFBS is here to assist you with your rehabilitation. Our goal is to get you back to work as soon as possible.

A successful rehabilitation is a team effort, with each member of the team responsible for different aspects of your recovery. The team includes:

  • You, as the injured eligible performer.
  • AFBS, who provides the benefits to help you recover.
  • The AFBS Medical Team from Oncidium who will assess your symptoms and guide you in taking immediate and appropriate steps towards a timely recovery.
  • Your Medical Team

If you are injured on set, you should:

  • First seek appropriate and immediate care;
  • Report the incident to the Production Manager and/or your on-set ACTRA representative.

Note: Once notified of your incident, the Production Manager must complete and submit Form 13-IAR to AFBS within 72 hours of the accident. If the Production Manager does not file the Form 13-IAR, contact your on-set ACTRA representative.

After Production Submits the Report

Once AFBS receives the report from production, AFBS will contact you by mail and email and request a consent form to obtain and exchange medical information. Once AFBS receives your completed and signed consent form, a dedicated nurse case manager from Oncidium will phone you to:

  • Learn about your injury.
  • Guide you in taking immediate and appropriate steps towards a timely recovery; and
  • Confirm your injury is the result of an accident sustained on a set location and provide a report to AFBS.

Checking Your Claim Status

In addition to mailed correspondence from AFBS as required, you’ll also be able to log in to the secure password protected claims portal on the Accident on Set website to check the status of your claim.

Receiving Benefits

Benefits will begin once:

  • AFBS receives and reviews the assessment report from Oncidium
  • AFBS has approved your claim, and
  • You have signed and provided AFBS with the release and waiver form (see below for release and waiver details).

Once approved, you will be notified by email.

Should you be eligible, benefits may include:

  • Weekly Income Replacement Benefits (WIRB) and Supplementary Health Care Benefits. Once approved and so long as you remain eligible, WIRB benefits are paid every two weeks. Your first WIRB payment will likely be larger as it includes payments for lost income from the date of injury to AFBS approval.
  • Supplementary Health Care Benefits are paid on reimbursement only. We aim to issue reimbursements within two weeks.
  • Accident Benefits are paid on an application and approval basis only.
  • All claim receipts must be mailed to AFBS. Emails or other forms of electronic submission are not accepted.

Release and Waiver Details

Entitlement to any eligible Accident on Set benefits payments requires that as the injured performer, you release the Producer from any liability as a result of an injury you sustain working on a set location on their Accident on Set-insured production by signing a release and waiver. The Accident on Set administrator will provide the release and waiver form, as well as any disability claim forms that may be required to assess your injury.
