Supplementary Health Care Benefits

Supplementary Health Care Benefits are available for health care expenses incurred by an eligible performer, as medically recommended, and as a direct result of an on-set location injury; such an injury must first be recognized and approved by AFBS as arising from the reported incident.

Performers and stunt performers may be eligible for Supplementary Health Care Benefits if a claim period has been approved and if they are receiving Weekly Income Replacement Benefits.

Ground Ambulance Services
  • To the nearest hospital or other emergency medical facility
  • Limited to the established co-payment amount in the injured performer’s province or territory of residence
Air Ambulance
  • For flights originating and terminating in Canada only
  • This Benefit does not cover repatriating injured performers to the province or territory where they reside
  • Performers must have out-of-province emergency coverage in place
  • Overall maximum reimbursement - $5,000

Hospitalization

The initial expense must be incurred for acute care in an accredited hospital. Subsequent room costs for rehabilitation in an accredited hospital are covered only if they immediately follow at least three days of acute care.

  • Room cost differential between ward and semi-private accommodation only
  • Does not cover standard ward accommodation costs
  • Payable within Canada only

Private Duty Nursing

(LPN, RN, RNA)

  • Must follow hospitalization
  • Must be doctor-recommended and pre-approved by AFBS
  • Up to $5,000 following each hospitalization
  • Overall maximum reimbursement - $25,000

Home Care Assistance

The provider must be supervised by an organization recognized as providing such care.

  • Must follow hospitalization and be used within 90 days
  • Must be doctor-recommended and pre-approved by AFBS
  • Up to $50 per day for a maximum of 21 days
  • Overall maximum reimbursement - $15,000

Accidental Dental

Excludes injuries to natural teeth caused by an object (wittingly or unwittingly) placed in the mouth, deterioration of teeth or by any other cause, except as indicated.

  • Injury must be due to breakage of tooth/teeth by a direct accidental blow to the mouth, or from the use of a dental prosthesis required by the production
  • Reimbursement is based on the current fee guide in the province or territory where the service is provided
  • Must be provided by a legally licensed dentist
  • Full treatment plan is required at the time of the accident

Prescription Drugs

The usual and customary ingredient cost and dispensing fee charges are covered.

  • Must be prescribed by a medical doctor, dentist or health care practitioner as specified under their provincially or territorially legislated scope of practice
  • Certain drugs are covered only after the injured performer submits a Special Authorization Request form and AFBS approves it
Vision Care
  • Examination and replacement of one pair of prescription glasses or contact lenses (up to a one-year supply) when needed as a result of the accident
  • If laser surgery is chosen as an alternative, the same maximum will apply
  • Sunglasses and safety glasses are excluded
  • Overall maximum reimbursement - $1,000

Part A

  • Chiropodist

  • Registered Psychologist
  • Performers are eligible for reimbursement once their medical doctor confirms the accident and AFBS approves it
  • Prior to reimbursement, the medical practitioner may be required to submit a treatment plan directly related to injuries the performer sustained
  • Up to $3,500* annually during the first 5 years
  • Up to $1,500* annually during the next five years

*Benefits for additional background performers and parents/guardians of a minor are limited to $1,000 annually for five years

Part B

  • Audiologist
  • Occupational Therapist
  • Physiotherapist
  • Registered Acupuncturist
  • Registered Massage Therapist
  • Speech Therapist
  • Requires a recommendation from the injured performer's medical doctor confirming both the necessity for treatment due to the accident, and the duration and frequency of expected treatment
  • Up to $3,500* annually during the first 5 years
  • Up to $1,500* annually during the next five years

*Benefits for additional background performers and parents/guardians of a minor are limited to $1,000 annually for five years

Medical Supplies

When prescribed by the injured performer’s medical doctor and after submission to any available provincial or territorial program.

  • AFBS pre-approval required for either rental or purchase
  • Limited to the specified amount or when unspecified, the usual and customary cost defined by AFBS
  • Limited to specified items
  • Abdominal, knee and/or back brace
  • Artificial eyes - one of each of left and right
  • Artificial limbs, including myoelectric prosthesis - overall maximum $20,000
  • Cast
  • Catheter
  • Cervical collar
  • Devices and medical aids needed after surgery
  • Hearing aids - annual maximum $500 per ear and overall maximum $2,000
  • Hospital bed - overall maximum $5,000
  • Ostomy supplies where medical stoma exists
  • Oxygen set
  • Splint
  • Tens machine - overall maximum $500
  • Tracheostoma tubes
  • Truss
  • Walking cane
  • Wheelchair - overall maximum $5,000

Corrective Footwear: custom-made boots or shoes, or adjustments to stock item boots or shoes.

These are covered only for a medical condition resulting from the qualifying accident.

  • Must be prescribed by either a chiropodist (D CH or D Pod M), medical doctor (MD) or podiatrist (DPM)
  • Must be dispensed by a pedorthist (C Ped(c) or C Ped (MC)) and include the biomechanical assessment, an itemized invoice listing all items and modifications, and detailing orthotic casting and fabrication methods including raw materials used
  • Overall maximum $5,000

Orthotics

Only available if a new medical condition results from a qualifying accident.

  • The performer's medical doctor must recommend and confirm the need for orthotics due to the accident
  • Must be prescribed by either a chiropodist (D CH or D Pod M), medical doctor (MD) or podiatrist (DPM)
  • Must be dispensed by either a chiropodist (D CH or D Pod M), orthotist (CO or CPO(c)), pedorthist (C Ped(c) or C Ped (MC)) or podiatrist (DPM)
  • Overall annual maximum $500

In the event of a discrepancy within this section, a misunderstanding or conflict, benefits will be paid according to the terms and conditions of the Master Insurance Policy (GH20000) and any applicable legislation.

Entitlement to any eligible Accident on Set benefits payments requires that as the injured performer, you release the Producer from any liability as a result of an injury you sustain working on a set location on their Accident on Set-insured production by signing a release and waiver. The Accident on Set administrator will provide the release and waiver form, as well as any disability claim forms that may be required to assess your injury.
